Salter has derived the concept of continuous passive motion (CPM) based on a series of experimental investigations. He followed a well out thought rationale: since immobilisation is unhealthy for joints and intermittent movement is healthier for both normal and injured joints, then continuous motion would be even better. Bearing in mind that one of the characteristics of skeletal muscle is fatigability, for motion to be continuous it should also be passive. Furthermore, CPM has another advantage in that it is slow enough which makes it applicable to patients immediately after operation or injury without causing pain.
CPM has found its use in rehabilitation of joints based primarily on the theory that joint motion promotes the healing and regeneration of articular cartilage. Joints that are prone to stiffness need early mobilisation after trauma or surgery, to avoid arthrofibrosis. CPM acts to reduce blood and fluid accumulation in and around joints thus reducing subsequent joint stiffness. Avoiding stiffness in the early stages diminishes the chances of a progression of a contracture in the joint.
CPM is nowadays provided through devices that are mechanically designed to perform all the movements that a specific joint is anatomically constructed to perform. They provide the opportunity to the therapist to set the rate by which the movement is performed as well as the force by which the movement is performed.
